Supports the physician and interdisciplinary team to facilitate patient care, while enhancing the quality of clinical outcomes, patient satisfaction and managing the cost of care. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. Through close collaboration with the CRMC Command Center, the RN Case Mgr incorporates admission status and medical necessity determination, to ensure that appropriate care and services are delivered efficiently and timely. /n/nQualifications:/n Experience and education minimum requirements - Bachelor’s degree in Nursing - One year acute care hospital experience. - Three years of acute care hospital experience may be substituted in lieu of education.
